HOME HEALTH SERVICES

  • Registered Nurses are available and on-call 24/7. Our nurses, during regularly scheduled visits, perform complete health assessments, medication review, and identify potential safety risks. They promote independence by teaching about health conditions, medications & side effects, and how to respond when acute symptoms arise. Nurses assist with wound care, catheter care, IV medications, pro-times for blood thinner usage, and more. The nurses stay in contact with your Primary Care Physician updating him/her on any changes. Our nurses help establish a sense of control and peace of mind for you, your family and friends.
  • Certified Nursing Assistants (CNA's ) are available to assist you with personal care. They have a caring and positive attitude with good interpersonal communication skills. They are trained to assist with grooming, hygiene, bathing, and dressing. They are also trained in skin care, applying compression stockings, urinary catheter care, and assist with linen changes. They practice effective infection control and pay attention to safe home practices. They report changes in health conditions to the nurse. 
  • Physical Therapy is an important aspect in your life to help improve and maintain motion and safe mobility in the comfort of your home. Movement is a very complex process involving nerve impulses, muscle contractions, free joint movement and coordination. Our goal is always to restore, adapt or improve your physical performance which may have been altered through an injury or illness. Our therapists also teach techniques to control pain with movement and improve overall health.
  • Occupational Therapists goals are to improve self care, restore function, and maintain home safety. Our therapists assist in restoring/improving Activities of Daily Living (ADL’s) through retraining (i.e., grooming, hygiene, bathing, dressing, feeding self); teaching care givers how to safely assist with ADL’s; and, teaching about home safety modifications that will improve your ability to function independently in the home.
  • Pulmonary Rehabilitation is a program for those suffering from chronic breathing problems. Our therapists help improve your ability to function and your quality of life through an individualized program of exercise training, energy conserving techniques, breathing strategies, and education on your lung disease. They will teach you how to respond to episodes of breathing difficulties by using your medications and breathing techniques for the best outcome. Improving your breathing function will reduce your trips to the Emergency Room and allow you to remain in control and comfortable in your own home.

    Therapy Referrals may include but are not limited to the following health conditions: Hip/Knee/Shoulder surgeries, Stroke, Parkinson’s,  Arthritis, Hand Injuries, Contractures, Neuropathies, COPD, Dementia, Brain Injuries (TBI), Amputations, Spinal Cord Injuries, or any Balance issues with frequent falls.
